What is HMIS?

The North Carolina Homeless Management Information System (NCHMIS) is a secure database used to collect and manage information about the services provided to individuals experiencing homelessness. HMIS is designed to improve service coordination, streamline access to resources, and help organizations like Homeward Bound understand and address homelessness in our community more effectively.

How We Use Your Information

We enter the information you provide to us during this interview into a database called the North Carolina Homeless Management Information System. All staff using the system are trained and bound by confidentiality and security rules. Homeless services organizations throughout North Carolina have access to the database, but they can only see your Profile Information that includes your name, date of birth, race, gender and the last four digits of your social security number. This information is used to find a record in NC HMIS and to make sure that you have only one record. The rest of the information you provide will only be seen by employees of Homeward Bound and requires a written agreement from you to be shared with another agency or a court order to be shared as required by law. Homeward Bound must report some demographic information because of our federal and state requirements, but your identifying information will be removed.

If there is any reason you believe having your Profile information visible to other agencies using this database would put you at risk, you have the option of restricting this Profile Information so that only Homeward Bound sees it, or entering it in as anonymous. At any point, you can get a copy of your record, have corrections made to your record, refuse to share your information with other agencies, stop your release of information, and file a grievance if necessary. You may also have a copy of Homeward Bound’s full Privacy Notice at any time.
